In order to receive services
(or continue receiving services) from HealthNet, patients
are required to go through an enrollment interview to
determine eligibility. This occurs
every Wednesday from 1:30p until 3:30p. Patients
are interviewed on a first come first served basis and
we will see as many individuals as we can. However,
please be aware that you may be asked to come back the
following week.
Please
bring the following documents with you to your enrollment
appointment! If you are missing documents, you
will NOT be able to enroll/re-enroll and will be asked
to return another week!
1)Proof
of residence - Driver's License or picture ID
2)Proof
of Residence - A piece of mail with your name and address
on it.
3)Employment
- If employed, bring in 2 current check stubs
4)Employment
- If you live with other individuals in the household,
please bring in 2 of their current pay stubs
5)Unemployemnt
letter showing weekly amount you are receiving
6)Federal
Medical Assistance or BadgerCare Denial Letter (must
apply at access.wi.gov and print denial letter)
7)Child
support letter with amount (if you are receiving child
support)
8)Food
stamp letter showing amount being received.
9)Rental
assistance letter with the amount.
10)Disability/Social
Security income: letter or bank statement showing deposit
11)Pension
income amount: letter or bank statement
12)Other
sources and amounts with verification.

& 2011
Taxes (page 1 only of 1040EZ) (Page 1 & 2 of 1040)
(Sched. C if self employed, 2 pages). ((2011 taxes will
not be due at HealthNet until April 30, 2012))

Our Services are contingent upon the
availablity of our volunteer doctors, nurses, and other volunteer
health care specialists. Services are only provided during clinic
hours.
